When administered to patients with acute cardiovascular pathology careful monitoring of hemodynamics is necessary due to the hypotensive effect. Use with caution in patients with blood pressure (BP) below 100 mm Hg, with myocardial infarction, multiple trauma, with anamnestic indications of liver and kidney diseases (pyelonephritis, glumerulonephritis), in the treatment of cytostatics, blood diseases. The risk of such reactions is also increased in patients with dehydration or dehydration, unstable blood pressure or incipient circulatory failure (eg, in patients with multiple injuries or heart attack), in patients with high temperature.

In such patients, careful examination and careful monitoring is required. Presumptive measures (such as circulatory stabilization) may be necessary to reduce the risk of hypotensive reactions.

In exceptional cases, the drug can be used in patients in whom lowering blood pressure is unacceptable, for example, in severe coronary disease heart or corresponding stenosis of the cerebral arteries. In such cases, the use of the drug is possible only after careful monitoring of hemodynamic parameters.

In patients with renal or hepatic insufficiency, metamizole sodium should be taken only after a careful assessment of the benefits, risks and necessary precautions.

With the simultaneous use of the drug with antispasmodics, sedatives, diuretics, the hypotensive effect is enhanced. The drug reduces the effect of dopegyt, increases the hypoglycemic activity of oral antidiabetic drugs, enhances the effect of ethanol, indirect anticoagulants, glucocorticosteroids (GCS) and indomethacin. With simultaneous use with cyclosporine, the concentration of the latter in the blood decreases. The effectiveness of Andipal is reduced by phenylbutazone, barbiturates and other inducers of microsomal liver enzymes; simultaneous appointment with other non-narcotic analgesics, tricyclic antidepressants, hormonal contraceptives and allopurinol leads to an increase in the toxic effect of andipal. Sedatives and tranquilizers enhance the analgesic effect of Andipal. Thiamazole and sarcolysin increase the risk of developing hematopoietic disorders. The effect of andipal is enhanced by codeine, H2 receptor blockers and propranolol. Simultaneous use with chlorpromazine or other phenothiazine derivatives can lead to the development of severe hypothermia; caution is necessary with the simultaneous use of the drug with radiopaque substances, colloidal blood substitutes and penicillin, procainamide, reserpine, quinidine, salicylates; it is possible to reduce the hypotensive effect of methyldopa and the antiparkinsonian effect of levodopa.

Side effects

From the gastrointestinal tract: nausea, constipation.

From the side of the central nervous system: drowsiness, increased sweating.

From the side of the cardiovascular system: violation of the heart rhythm, a decrease in cardiac output, as a result of which deterioration in the electrocardiogram is possible.

Allergic reactions: skin rash, itching, urticaria, angioedema.

Metamizole sodium :

Allergic reactions. Uncommon: rash; rarely: maculopapular exanthema, anaphylactic or anaphylactoid reactions. They occur mainly within the first hour after ingestion. Usually manifested in the form of skin reactions, such as itching, burning, redness, rash, swelling, rarely - shortness of breath. May occur with more severe forms, such as: urticaria, severe Quincke's edema (including the larynx), severe bronchospasm, heart rhythm disturbance, arterial hypotension (sometimes preceded by an increase in blood pressure); very rare: analgesic-induced bronchial asthma(including in patients with aspirin asthma), Stevens-Johnson syndrome or Lyell's syndrome, shock.

Reception of metamizole sodium with the appearance of skin reactions should be immediately discontinued.

Blood and lymphatic system. Rare: leukopenia, agranulocytosis or thrombocytopenia. These reactions are probably immunological. They can occur even if metamizole sodium has previously been used without complications. The risk of developing agranulocytosis may be increased if metamizole sodium is taken for more than 1 week.

Agranulocytosis is manifested by high fever, chills, sore throat, difficulty swallowing, inflammation of the mouth, nose, throat, and genitals, or anal itching. The erythrocyte sedimentation rate (ESR) is significantly accelerated, the number of granulocytes is significantly reduced or completely absent. As a rule, but not always, the values ​​of hemoglobin, erythrocytes and platelets remain within the normal range.

Reception of metamizole sodium should be stopped immediately, even in the absence of the results of laboratory diagnostic tests, subject to a sudden deterioration in the general condition of the patient.

The cardiovascular system. Infrequently: hypotensive reactions during or after the use of the drug. Even with hyperthermia, a decrease in blood pressure can occur without any signs of a hypersensitivity reaction.

Diseases of the kidneys and urinary tract. Very rarely: acute deterioration of renal function, after which proteinuria, oligo- or anuria and acute renal failure, acute interstitial nephritis may develop. Red staining of urine.

If side effects or other unusual reactions occur, consult your doctor about further use of the drug.

Overdose

Metamizole poisoning symptoms: hypothermia, palpitations, shortness of breath, tinnitus, nausea, vomiting, weakness, drowsiness, impaired consciousness, convulsive syndrome; development of acute agrunolocytosis, acute renal and hepatic insufficiency is possible. Symptoms of papaverine poisoning: diplopia, weakness, drowsiness, lethargy. Symptoms of bendazol poisoning: arterial hypotension, headache, weakness.

Treatment: drug withdrawal, gastric lavage, administration of activated charcoal, symptomatic therapy aimed at maintaining vital functions. The main metabolite of metamizole - 4 N-methyl-amino-antipyrine - can be removed by hemodialysis. The treatment of intoxication, as well as the prevention of serious complications, require intensive medical supervision and treatment.
